Submit a Claim - Claim Instructions

If you had an eligible claim in connection with the related Class Action, In Re Baxter International Inc. Securities Litigation, Case No. 1:19‑cv‑07786, and received a Plan Notice with a Tax Certification Form, you do not need to submit a Claim Form now, unless you had additional purchases between May 6, 2015, and October 23, 2019, or otherwise want to modify your claim, but you must return the Taxpayer Certification Form included with the Plan Notice. ALL claims, claim modifications, and TAX documents MUST BE submitted so they are postmarked or received by the Distribution Agent on or before March 8, 2024.

If you had a claim in connection with the Class Action that was denied for not having valid documentation supporting the purchases/acquisitions of Baxter International common stock claimed, or the Claim Form is missing (i) a valid signature; or (ii) the authority and/or the capacity of the person to sign on behalf of the beneficial owner, you must submit documentation to resolve your deficiency so that it is postmarked or received on or before March 8, 2024 (“Claims Bar Date”).
